I had a heart attack, and my doctor told me that it was definitely stress-related. I know that the company has been nothing but kind and accepting as far as my care has been concerned, but I am afraid it won’t be enough.
My doctor has requested that I take some time off and relax. He demands that I leave work because in his opinion it is too taxing for someone like me at the moment. I wish there was a way to continue working, because I see the company as an extension of myself, and now just an occupation, but my hands are tied.

My reason for informing you regarding this was to tell you that I would no longer be continuing with you and the company. I cannot tell you just how disheartening it is for me to make this decision, and I hope I can come back and work with you in the future.
